chase modification help

I am in desperate need for help. We are currently 90 days late on our mortgage, due to financial hardships. we have contacted chase and let them know about our situation. My husband spoke to someone who had said that we need to send 2 months worth of bank statements, pay stubs and and expense sheet. He also said that we need to make a payment $2,635.00 and that he needed a bank acct # and routing number by today or the foreclosure process will start. Does this sound like loss mit or collections? we are in desperate need for help.:mad:
