Default Forged TILA -Truth in Lending Act

I received a QWR without pymt history of course! Anyways looking over the papers, the first TILA is forged with my name and my initials are also forged and there's a bunch of blanks where a signature should have been. In addition, there are two fax times on the top of front page with no phone numbers! I did not sign them. I was laid up in a hospital in another state on morphine and oxycodone and was in no condition to sign them. I have proof of all of this. I am thinking that I can probably get title to my house free and clear once this is proved. Does anyone know anything about this? I have been fighting my foreclosure pro se for the last 8 mths. I cannot afford an attorney.
